- Frequency Limitation
- 1 per quadrant every 24 consecutive months (ROLLING_MONTHS)
- Age Limitation
- Adults or documented early-onset periodontitis.
Required Claim Attachments & Pre-Submission Checklist
- Full-mouth 6-point periodontal probing chart (probing depths >= 4mm clearly shown)
- Diagnostic bitewing or periapical radiographs showing alveolar crestal bone loss and subgingival calculus
- Documentation of active bleeding on probing
Bill When (Allowable Clinical Criteria)
- 4 or more teeth in the quadrant have probing depths >= 4mm with clinical attachment loss and bleeding
- Radiographic bone loss is clearly visible
- Therapeutic treatment of diagnosed periodontitis
Restrictions & Clinical Exclusions
- Do NOT bill on same date as routine cleaning (D1110)
- Do NOT bill if only 1 to 3 teeth affected in quadrant (use D4342 instead)
- Do NOT bill within 24 months by same dental office
