- 2 per calendar year or 1 every 6 months (CALENDAR_YEAR)
- Age Limitation
- Commercial PPO typically covered through age 13 (cut off at age 14). Medicaid / CHIP typically covers through age 18 (cut off at age 19).

- Do NOT bill separate fee if using fluoride-containing prophylaxis paste during cleaning
- Do NOT bill for commercial adult patients without high caries risk rider
